Instructions to download PEQ FORMS :
1) Please download the forms by clicking on the link above.
2) File will start to download and open the Word Application Automatically.
3) Once the file is open fill up the necessary information.
3) Save the file in your work folder e,g, My Document. by clicking on the "File" menu and
Select "Save as" from the menu, give the relevant file name (e.g. PEQ-YOURNAME.DOC)
4) email us the file as an attachment.
